ABOUT THE FORMS
These forms were designed to the specifications of a professional human resources management consultant. Although developed for a retail/services business, they should be appropriate for most companies that understand the need for an objective, written basis for critical personnel decisions. Before firing an employee for poor performance, for example, a history of “unsatisfactory” appraisals and unmet improvement goals should be demonstrated in order to avoid complaints of unjust dismissal. Likewise, a company may be exposed to charges of discrimination by promoting a non-minority in the absence of a record that documents performance that justifies the promotion of the selected individual as compared to other candidates.
The three forms in this package are:
(1) An Activity Report that provides a means by which the employee can summarize his/her achievements with respect to job responsibilities and/or goals during the review period. There is also space for the employee to note special contributions outside normal job parameters.
(2) A Performance Appraisal form used to rate and comment upon an employee’s performance in key areas. These areas are generic and should be applicable to virtually any work environment and position.
(3) A Managerial Performance Appraisal form used to rate and comment upon managers’ performance in distinctly supervisory/managerial functions, such as budgeting. This form is intended to be used •in addition to• the generic Performance Appraisal form used for all personnel.
All three forms are single page, double-sided.
PRINTING THE FORMS
The forms are in Macintosh PageMaker 4.2 format. For maximum “transportability,” Times is the font used for all text, except the copyright notice and document number at the bottom of each page; these are in Helvetica.
For the checkboxes on forms (2) and (3) I used the ‘r’ character from the BillsDingbats shareware font, available on America Online and other services. If you do not have BillsDingbats you can employ other common dingbat fonts as follows:
• DeltaSymbol: ‘g’ (This font is distributed with DeltaPoint’s Deltagraph application.)
• Wingdings: ‘o’ (heavy border) or Option-‘r’ (light border)
• ZapfDingbats has only shadow boxes and a filled box—the character ‘n’. You can use this character, however, to make an “empty” checkbox: just apply the outline style to the character.
The forms contain no PostScript objects as such and no grayscale, and thus can be printed on QuickDraw as well as PostScript devices. Nevertheless, I recommend a 300 dpi or better PostScript printer for best results.
